What is the DocuSign form that I am being asked to submit?
The DocuSign is a HIPAA form that serves as a standard intake document, helping both patients and providers understand key guidelines. It does three things: it explains patients’ rights and provider responsibilities, protects providers from unfair complaints, and follows the law to ensure clear communication between both parties. As well as providing any medical history.
A licensed provider will review your details to ensure the best treatment plan for you, usually within a few minutes based on your response time.
Common blood tests include A1C, CMP, CBC, TSH, and Lipids.
If you’ve had these labs drawn within the past 12 months, they’ll likely be accepted. Bloodwork is based on your needs, and if necessary, your provider will give you a script during your consultation.
Most insurance plans cover lab work. If you don’t have insurance, we offer a $57 flat-rate deal with Ulta Labs (excluding RI, NY, and NJ).

Once you have completed your consultation phase with the provider, it should take about 7-10 business days to receive your treatment(s). Overnight options are available.
Are there shipping charges?
Shipping and handling typically average $30 per shipment nationwide and are not included in the medication cost
Do you ship to my state?
Our medications and peptides come only from US State Licensed pharmacies and are prescribed by certified healthcare providers. These pharmacies can produce treatments at more affordable dosages than retail pharmacies, allowing us to offer cost savings and ensure a steady supply of treatments.
